How the APGAR score works

The APGAR score is a structured 0–10 tally of five newborn signs (Appearance, Pulse, Grimace, Activity, Respiration), each scored 0, 1, or 2, recorded at 1 and 5 minutes (and every 5 minutes thereafter while the score stays 7 or below). It documents the newborn's condition for the record; resuscitation, when needed, starts before the 1-minute score is assigned and is never driven by the number.

Each total maps to an interpretive band: 7–10 reassuring, 4–6 moderately abnormal, 0–3 low. The component anchors and bands come from StatPearls (NBK470569), not from us, and the band is context for the record rather than a disposition.

Frequently asked questions

How is the APGAR score calculated?
Each of the five signs (Appearance, Pulse, Grimace, Activity, Respiration) is scored 0, 1, or 2, and the five are summed to a total of 0 to 10 at each time point. The score is recorded at 1 minute and 5 minutes, and every 5 minutes after that while it stays at 7 or below.
What do the APGAR bands mean?
As interpretive context only: 7–10 is reassuring, 4–6 is moderately abnormal, and 0–3 is low. The bands are not a diagnosis or a disposition.
Does a low APGAR mean birth asphyxia?
No. An APGAR score is not evidence of asphyxia and does not predict long-term neurologic outcome. It is a documentation aid interpreted alongside the full clinical picture.
Does the score decide resuscitation?
No. Resuscitation is guided by the newborn's condition and begins before the 1-minute APGAR is even assigned. The tool recommends nothing.
Are APGAR entries sent anywhere?
