Opening a clinic in India is a stack of separate registrations from different authorities, and which ones apply depends on what you do and where you practise. Use this as a working checklist, not a guarantee: requirements, forms, and fees vary by state and municipality.

Before you start

  • Each treating doctor holds a current State Medical Council registration.
  • Premises sorted (ownership or registered lease, suitable for clinical use).

Core registrations (most clinics)

  • Clinical establishment registrationCentral Clinical Establishments Act in adopting states; own state law elsewhere (e.g. Maharashtra). Confirm your state.
  • Local trade / shop-and-establishment licenceFrom your municipality; name and process vary by city.
  • Biomedical waste authorisation + CBWTF tie-upSegregate and dispose under the BMW Rules, usually via a common treatment facility.
  • Fire safety NOC (where the premises/occupancy requires it)

Only if they apply

  • Retail drug licenceIf you stock and dispense medicines like a pharmacy. A doctor's own dispensing to their patients is a narrow exemption.
  • PCPNDT registrationIf you have an ultrasound or any prenatal-diagnostic imaging machine. Register before using it.
  • GST registrationIf you make taxable supplies (e.g. sell medicines) or cross the turnover threshold. Pure clinical services are largely exempt.

Ongoing

  • Diarise every renewal date (registrations and licences expire).
  • Display registration certificates as required by your state.
  • Keep the records each regime asks for (waste manifests, Form F, etc.).
