There is no single "clinic license" in India. Opening a clinic means clearing a handful of separate registrations, each from a different authority, each on its own timeline. Some are central, most are run by your state or municipality. This is the full list, who needs each, and where to go deep.

The "issuing authority" column matters more than it looks. Anything marked State or Municipal varies by where you are. Karnataka runs clinical establishment registration through an online state portal; Maharashtra registers clinics under its own Bombay Nursing Homes Act instead of the central law; Delhi and Tamil Nadu run their own systems too. So the same row means a different office and a different form depending on your city.

Documents you'll need, by registration

Most of these registrations ask for the same core set, plus a few specific to the license:

  • Identity and entity: PAN of the clinic or proprietor, the business registration certificate, and address proof.
  • Premises: ownership papers or a registered lease deed, the building completion or occupancy proof, and a layout plan.
  • People: the doctor's medical council registration, qualification certificates, and the same for any nurses or technicians.
  • Specific add-ons: biomedical waste needs your tie-up agreement with a common treatment facility; a drug license needs the pharmacist's details; a PCPNDT registration needs the ultrasound machine details and the operator's qualifications.

Keep one folder with the core set scanned and ready. You will hand the same documents to four different offices.

A small clinic vs a bigger one

Not every clinic needs every row. The shape of your clinic decides the list:

  • A single-doctor consulting clinic needs clinical establishment registration (where the state requires it), professional registration, and basic biomedical waste handling. It often needs nothing else.
  • Add a pharmacy counter and you pick up a retail drug license and, usually, GST registration once the taxable sales cross the threshold.
  • Add an ultrasound or imaging and PCPNDT registration becomes mandatory, with serious penalties for skipping it.
  • Add X-ray and you need an AERB license on top.

So the honest answer to "what licenses do I need" is "it depends on what your clinic does." Build the list from your services, not from a generic checklist.

No. A small single-doctor clinic that only consults and treats needs far fewer than a clinic with a pharmacy, an ultrasound, or X-ray. The core ones are clinical establishment registration (where your state requires it), biomedical waste handling, and professional registration. The rest depend on what you actually do.
Which licenses are state-specific?
Most of them. Clinical establishment registration, the shops and establishment registration, fire NOC, trade license, and the drug license are all administered by your state or municipal body, and the process and fees differ. Treat any national checklist, including this one, as a starting map, not the final word for your state.
Do I need a drug license if I only give medicines during treatment?
Often not. Medicines administered as part of immediate treatment at your own clinic usually fall under an exemption. The moment you stock and sell medicines like a pharmacy, you need a retail drug license. The boundary is state-specific, so confirm with your state drugs control office.
What happens if I run a clinic without registering it?
