The "issuing authority" column matters more than it looks. Anything marked State or Municipal varies by where you are. Karnataka runs clinical establishment registration through an online state portal; Maharashtra registers clinics under its own Bombay Nursing Homes Act instead of the central law; Delhi and Tamil Nadu run their own systems too.
