The "issuing authority" column matters more than it looks. Anything marked State or Municipal varies by where you are. Karnataka runs clinical establishment registration through an online state portal; Maharashtra registers clinics under its own Bombay Nursing Homes Act instead of the central law; Delhi and Tamil Nadu run their own systems too. - Identity and entity: PAN of the clinic or proprietor, the business registration certificate, and address proof.
- Premises: ownership papers or a registered lease deed, the building completion or occupancy proof, and a layout plan.
- People: the doctor's medical council registration, qualification certificates, and the same for any nurses or technicians.
- Specific add-ons: biomedical waste needs your tie-up agreement with a common treatment facility; a drug license needs the pharmacist's details; a PCPNDT registration needs the ultrasound machine details and the operator's qualifications.
